CARE MANAGEMENT SERVICES
Consultation
Geriatric Support/Pathway Care staff are
available to meet with you to discuss your concerns regarding your elderly or
frail loved ones. Specific issues which families are sometimes concerned with
include:
- How do I know if I should be concerned about
my loved ones?
- How do I talk with my family member about
these concerns?
- Who can help us plan for future legal,
financial and care needs?
- What services are available to my parents,
and will meet their needs?
Assessment
Geriatric Support/Pathway Care conducts an
initial comprehensive evaluation of the older adult’s strengths and
limitations, including:
- Physical health status
- Cognitive and psychological status
- Current home environment
- Functional abilities and limitations
- Social interactions
- Legal/financial planning
Care Plan Development & Implementation
Based on the assessment, Geriatric
Support/Pathway Care develops a personalized plan of care to fit your family’s
situation. Services we can help you obtain include:
- In-home and community-based care such as
in-home caregivers, delivered meals, chore services, transportation, and
adult day programs.
- Alternative living arrangements, including
retirement centers, assisted living settings, group homes, and nursing
homes.
- Assistance with obtaining appropriate
legal/financial services, including: benefit eligibility, financial
management and planning, and legal matters (power of attorney,
guardianship).
- Referral to medical and psychiatric
assessment and treatment resources.
Ongoing Follow-Up
Your individualized plan of care will meet your
family’s needs today. Geriatric Support/Pathway Care will then be available to
provide ongoing follow-up, monitoring, consultation, and advocacy to ensure that
your loved one continues to receive high-quality, appropriate care, and to
adjust the care plan as the needs change.
These services include:
- Supervision and monitoring of in-home care.
- “Friendly visitor” service to provide
monitoring, support, and socialization for those in their own home or
residential settings.
- Ongoing support and consultation for family
members.
- Coordination of medical, health care, and
other support services.
