| Positions available: part-time care managers Geriatric Support Associates, independent care management agency (established 1988), is looking for one or more part-time care managers to accommodate a growing caseload. If you are looking for a supportive, flexible, work environment which will provide opportunities for professional growth and development, this could be the job for you.
Qualifications
•Bachelor's degree in relevant field such as social work, counseling, geriatrics, or nursing. Master’s degree preferred.
•Minimum 3 years experience working with the elderly.
•Experience with complex family situations.
•Self-starter, ability to work independently, and seek consultation as needed.
•Familiarity with local resources (one or more counties in Southeastern Wisconsin: Milwaukee, Ozaukee, Washington, Waukesha, Racine, Kenosha)
•Interest in working part-time, flexible hours, with on-call availability. (Initially 2-3 days/week, with potential for growth).
•Wisconsin driver's license, current auto insurance, and reliable vehicle available for client visits
•Computer knowledge including ability to work with MS Office and learn new software programs. Computer and internet access at home, to enable regular use of email and internet programs.
Responsibilities
•Comprehensive assessment (cognitive, functional, psycho-social, health, legal/financial)
•Development & implementation of client & family-centered plan of care
•Communication and advocacy with medical and service providers
•Support and assistance for families, both local and long-distance
